Application for a theater license shall be made in writing under oath and shall set forth such information in such form as may be prescribed by the city clerk. The applicant shall pay the city clerk a fee of $10.00 for investigating the application and the application shall be accompanied by the additional sum of $50.00 as an annual license fee for that period terminating on July 1st following the date of application.
