The fee, for such license, shall be ten dollars for the giving or holding of a single dance, and forty dollars for an annual permit for every calendar year, or part thereof, and said license shall be issued in the name of the person, firm or corporation operating the establishment in which such dancing is to be done, and shall be valid only in such establishment.
